All CASMAN students must complete a lunch application form for the
2022-2023 School Year.
Instructions on how to fill out the form are on page 2 of the form.
How to Fill Out the PDF & Send It Back
1. Open the PDF document linked below.
2. Fill in the necessary form fields on the PDF.
3. Save the completed PDF to your device.
4. Open your personal email account and compose a new message to cturk@manistee.org
5. Click the Attach files (Paperclip) button.
6. Browse to where you saved the PDF file, then double click on the file to attach it.
7. Click the Send button.
